KINDLY DO NOT SUBMIT ANY PAYMENTS WITHOUT FIRST SCHEDULING YOUR APPOINTMENT. We ask that you provide payment for any telephone consultations at least 48 hours prior to your scheduled appointment. Patients who provide at least 24 hours notice of cancellation will receive a credit for rescheduling their phone consultation. The first selection below is only for telephone health counseling regarding a single issue of concern. The second selection is for current patients in need of a direct phone conversation between regular visits. Please use this page only for these two purposes–thank you!
